Sunnyvale School Clinic News FLU VACINE Its fall and time to think about getting your flu shot. We will be having our annual flu vaccine clinic at the school on October 29th, from 10am to 2pm. Cost is $30.00. If you cant make it call your Dr. for your flu vaccine. Maxum Health will be set up at the elementary school to administer flu vaccine. IMMUNIZATIONS If you receive a letter from the nurse regarding your childs immunizations please return the information requested ASAP. Most of the students will be due a tetanus/diphtheria (Td or Tdap) booster. If your child has had a Td booster in the last 5 years we may not have the date and may only need documentation from your physician/clinic that administered the vaccine. Grown ups need shots, too!! If you have not had a Td for 10 years or more you need a booster as well. Most adults will be receiving the Tdap vaccine. The p part of the vaccine is for Pertusis or Whooping Cough. This is currently being added at a reduced level because of frequent outbreaks in the older adult population.
Vision & Hearing Screening You may receive a referral concerning vision/hearing screening. If your child did not wear their glasses that day they will be rescreened at a future time. If this was the second time they failed screening you will receive a letter referring you to your physician for follow up care. We do need a letter from the Dr. doing the follow up care noting the plan of treatment for your child. This is a requirement by the State of Texas. MEDICATIONS We can not give over the counter medications, even Tylenol, without written permission from you and your Dr. You can have your Dr. fax the information to us with the dosage, how often it is to be taken & for how long. No medication can be given without the Dr. & the parents written consent for OTC meds. Forms are available in the clinic. If it is a prescribed medication the pharmacy label must accompany the medication & we need you to sign a form so we can administer the medication. ILLNESS If a child is sent home sick or has been sick. It is recommended that the child does not return until they are: fever free for 24 hours; on an antibiotic for 24 hr.; or the Dr. gives the child permission to return in writing. This is primarily to prevent the spread of the illness. If you have questions please call us at any time. Please encourage frequent and good hand washing. Remind your child no sharing or drinking after others. Do not share combs, brushes or makeup, & cover the mouth and nose when sneezing &/or coughing. 2007-08 Texas Minimum State Vaccine Requirements for Student LOW COST CLINICS.doc Screenings.doc Medication Policy.htm Head Lice.pdf HEAD LICE SCHOOL POLICY.doc When your child is ill.doc Over-the-counter Medication.htm |
